The Center's physicians, neuropsychologists, and therapists work together to meet the individualized rehabilitation needs of each patient.
These licensed specialists assess current functional skills and make recommendations in order to help restore the skills needed to resume daily school, work, household, and leisure activities. Cognitive and neurobehavioral skills are evaluated as well. Recreation programs, educational services, homemaking skills, and driver screening assessments that reinforce independent living are just some of the many services found at the Center.
Our Brain Injury Outpatient Rehabilitation Program is accredited by CARF (the Rehabilitation Accreditation Commission).
Environmental Assessments
Therapists also make home, school, and work visits, assessing the need for specific environmental modifications and adaptive equipment. Suggested modifications can range from simple changes such as rearranging furniture, removing "throw rugs," and installing a grab bar; to complex suggestions such as remodeling a bathroom, kitchen, or worksite for wheelchair accessibility, or installing an electronic environmental control system.
Rehabilitation Tailored to Individual Need
Individuals coming from an acute rehabilitation hospital program, from an acute care hospital, or from their primary care/specialist physician receive services tailored to meet their specific needs. Because of the comprehensive nature of the services provided by the Center, patients are often discharged from the hospital sooner and begin the highly intensive rehabilitation regimens that maximize functional independence on an outpatient basis. Patients requiring less intensive rehabilitation can utilize the neuropsychologist and occupational, physical, or speech-language therapies, or therapeutic recreation specialists on an "as-needed" basis. Specialized rehabilitative services for pediatric patients are also available.
Working Towards Successful Re-Integration
Continuity of care is a hallmark of Unity Comprehensive Rehabilitation Center. Patients discharged from Unity Acute Rehabilitation & Brain Injury Program have the advantage of the same physician following their care as they progress toward their goal of successful reintegration into their work and home lives. All patients at the Center benefit from the efforts of the case management staff as they work toward coordinating rehabilitative care and promoting communication between provider, primary care physician, patient, and family.
Convenience and Responsiveness
Unity Comprehensive Rehabilitation Center is conveniently located. Patients may also utilize our other rehabilitation sites located in Irondequoit, Brighton, and Greece.
