New York State Nursing Home Transition and Diversion Medicaid Waiver Program
The Nursing Home Transition and Diversion Medicaid Waiver Program (NHTD) was created to offer New Yorkers with disabilities and senior citizens a way to live in the community. The NHTD program offers a wide range of special services that will help people in their daily lives. These services include re-teaching basic skills, building ramps to allow access to homes, and safety monitoring.
Unity Health System is one of nine organizations designated as a Regional Resource Development Center in New York State that can help those interested in the NHTD program.
Our regional resource development specialists promote participant choice, endure the delivery of high quality services, and assist in the development of additional community services.
Home and Community Based Supports at Unity Health System:
- Serve as an educational and referral resource for participants and their families
- Work to develop community services and resources
- Interview and perform preliminary assessments of potential waiver participants
Eligibility Requirements:
The program is for people who choose to live in the community instead of a nursing home and who meet these requirements.
- 18 years of age or older
- Eligible for nursing home level of care
- Eligible for Medicaid coverage for community based long term care
How does the program work?
Unity will work with the applicant to choose a service coordinator who will help them apply for the NHTD program and to develop a service plan. The service plan is designated to help the person live safely in the community.
Unity Health System serves as the Regional Resource Development Center for Genesee, Livingston, Monroe, Ontario, Seneca, Wayne, and Yates counties.
